Everybody SHOULD have this form filled out and in the CAR
with them as they travel
with their dogs ... they will call an ambulance for you to go to hospital ... and the dog pound to take you animal away ...
UNLESS ... you tell them via this form ... to call a RELATIVE ... or someone to come and get the dog in case of emergency !
